When you talk about open world sandbox RPG's you must mention games series like Witcher and Elder Scrolls, there is one game that tried to be like those two series, it didn't quite manage to get the popularity of those games, but it was a decent RPG called Two Worlds 2. Game was released in 2010 by Polish development studio Reality Pump. Game is set in the fantasy world of Antaloor where you as a hero need to save the world from the hordes. I won't go too into story because I don't want to spoil it for those who didn't play this great RPG.
This open world RPG will take you to many great looking locations and environments. You will be fighting many various beasts and monsters. Customizing you character is done great in this game with great and detailed armor sets and weapons. Game features innovative magic system that let's you customize spells. Two Worlds 2 has robust and detailed crafting system where in order to get some great results you would need to explore the wilderness for rare materials. This is a truly hardcore RPG that I would recommend to any fan of Elder Scrolls games willing to play someting new.
